Mae rheoli perfformiad yn cynnwys nodi&#8217;r safonau perfformiad a ddisgwylir a chefnogi a rheoli staff i gyflawni eu gwaith.<\/p>\n\n\n\n\n\n<p>Mae [ENW&#8217;R MUDIAD] wedi ymrwymo i sicrhau bod yr holl staff yn cael arfarniad blynyddol, pan fyddwn yn trafod perfformiad gwaith, yn cynnig adborth ar waith a&#8217;r hyn a gyflawnwyd, yn cynllunio amcanion gwaith a gweithgareddau ar gyfer y flwyddyn i ddod ac yn cytuno ar y cynllun dysgu a datblygu.<\/p>\n\n\n\n<p>Mae&#8217;r polisi hwn yn nodi diben yr arfarniad a sut y caiff ei gynnal yn [ENW&#8217;R MUDIAD]<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"cyfrifoldeb\">Cyfrifoldeb<\/h2>\n\n\n\n<p>Cyfrifoldeb y rheolwr dynodedig yw sicrhau bod yr arfarniad blynyddol yn cael ei gynnal ar gyfer yr holl staff y mae\u2019n ei reoli. Cyfrifoldeb y ddau barti yw paratoi ar gyfer yr arfarniad, gan ei gwneud yn sesiwn bwrpasol ac effeithiol. Mae gan y ddwy ochr gyfrifoldebau i weithredu unrhyw gamau gweithredu dilynol y cytunwyd arnynt.<\/p>\n\n\n\n\n\n<h2 class=\"wp-block-heading\" id=\"beth-yw-arfarnu\">Beth yw arfarnu?<\/h2>\n\n\n\n<p>Mae goruchwyliaeth yn canolbwyntio ar faterion gwaith o ddydd i ddydd neu o wythnos i wythnos. Mae arfarnu&#8217;n digwydd unwaith y flwyddyn ac mae&#8217;n cymryd golwg tymor hwy. Mae arfarniadau yn agwedd annatod ar reoli effeithiol yn [ENW&#8217;R MUDIAD].<\/p>\n\n\n\n\n\n<h2 class=\"wp-block-heading\" id=\"nodaur-arfarniad\">Nodau&#8217;r arfarniad<\/h2>\n\n\n\n<p>Nodau ein proses arfarnu yw:<br><em>[dileer fel y bo&#8217;n briodol yn seiliedig ar fformat cynllun arfarnu eich mudiad]<\/em><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Adolygu a diweddaru&#8217;r disgrifiad swydd &#8211; lefelau cyfrifoldeb a chynnwys<\/li>\n\n\n\n<li>Darparu cyswllt rhwng nodau strategol [ENW&#8217;R MUDIAD] a r\u00f4l ac amcanion gwaith pob unigolyn<\/li>\n\n\n\n<li>Darparu strwythur ffurfiol i drafod sut mae staff yn teimlo am eu swydd a&#8217;u cyfrifoldebau<\/li>\n\n\n\n<li>Rhoi adborth ffurfiol ar gynnydd a pherfformiad gwaith<\/li>\n\n\n\n<li>Trafod a chytuno ar amcanion gwaith y flwyddyn nesaf<\/li>\n\n\n\n<li>Gwella gweithio mewn t\u00eem<\/li>\n\n\n\n<li>Rhoi cyfle i sicrhau bod staff yn hyderus ynghylch cymhwyso polis\u00efau a gweithdrefnau&#8217;r mudiad<\/li>\n\n\n\n<li>Rhoi cyfle i nodi anghenion datblygu a chytuno ar y cynllun dysgu a datblygu<\/li>\n\n\n\n<li>Adolygu effeithiolrwydd goruchwyliaeth a rheoli llinell, gan edrych ar yr hyn sy&#8217;n gweithio&#8217;n dda ac a oes unrhyw bryderon.<\/li>\n\n\n\n<li>Sicrhau bod y broses hon yn hygyrch i\u2019r holl staff, gan gynnwys y rhai sy\u2019n gweithio o bell neu sydd ag anghenion cymorth ychwanegol.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"manteision-arfarnu\">Manteision arfarnu<\/h2>\n\n\n\n<p>Mae arfarniad staff yn cynnig:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Eglurder ynghylch sut mae eu swydd yn cysylltu \u00e2 nodau strategol y mudiad, gan sicrhau bod pawb yn mynd i&#8217;r un cyfeiriad<\/li>\n\n\n\n<li>Cyfleoedd i deimlo&#8217;n gysylltiedig, gan arwain at fwy o gydlyniant a gweithio\u2019n well mewn t\u00eem ar draws y mudiad<\/li>\n\n\n\n<li>Cyfle ffurfiol i dderbyn cydnabyddiaeth ac adborth am eu gwaith a&#8217;u cyfraniad<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"agenda-arfarnu\">Agenda arfarnu<\/h2>\n\n\n\n<p>Mae arfarniad blynyddol yn rhoi cyfle i staff gyfarfod \u00e2&#8217;u rheolwr llinell ar sail un i un i adolygu&#8217;r flwyddyn ddiwethaf a chynllunio&#8217;r flwyddyn i ddod. Diben yr arfarniad yw:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Amcanion gwaith<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Monitro cynnydd yn erbyn y disgrifiad swydd ac amcanion gwaith mesuradwy y llynedd<\/li>\n\n\n\n<li>Rhoi adborth adeiladol ar y gwaith a gyflawnwyd a pherfformiad a chyfraniad cyffredinol yn ystod y flwyddyn<\/li>\n\n\n\n<li>Cynllunio amcanion gwaith y flwyddyn nesaf<\/li>\n\n\n\n<li>Cytuno ar unrhyw newidiadau i&#8217;r disgrifiad swydd<\/li>\n\n\n\n<li>Trafod unrhyw heriau sy\u2019n gysylltiedig \u00e2 threfniadau gweithio hybrid, o bell neu hyblyg.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Cynllun dysgu a datblygu<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Adolygu\u2019r amcanion dysgu a\u2019r datblygu a gyflawnwyd y llynedd<\/li>\n\n\n\n<li>Nodi unrhyw feysydd i&#8217;w gwella ac anghenion hyfforddi yn y dyfodol<\/li>\n\n\n\n<li>Cytuno ar flaenoriaethau hyfforddi a datblygu&#8217;r flwyddyn nesaf<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Amserlen<\/h3>\n\n\n\n<p>Bydd yr arfarniad blynyddol yn cael ei gynnal yn ystod mis [MIS]. Mae hyn er mwyn caniat\u00e1u i&#8217;r broses arfarnu fod yn gysylltiedig \u00e2&#8217;r cylch cynllunio blynyddol.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Ffurflen Cadw Cofnodion ac Arfarnu<\/h3>\n\n\n\n<p>Dangosir y ffurflen Arfarnu yn <a href=\"#atodiad-1\">Atodiad 1<\/a>. Fe&#8217;i defnyddir i baratoi ar gyfer yr arfarniad ac i gofnodi&#8217;r camau gweithredu o&#8217;r cyfarfod. Dylai&#8217;r ddwy ochr ddarllen (a llofnodi) y ffurflen ar \u00f4l ei chwblhau fel cofnod gwir a chywir o&#8217;r arfarniad.<\/p>\n\n\n\n<p>Bydd y ddwy ochr yn cadw copi o&#8217;r ffurflen.
